A leading rehabilitation and nursing care facility.

The high-quality care provided at The Manor Health and Rehabilitation Center at CentraState has been recognized through numerous honors and designations, including:

Best nursing home.

The Manor has been named one of the Best Nursing Homes by U.S. News & World Report, a nationally recognized magazine that routinely evaluates and rates health care providers across the country. The Manor earned this designation by demonstrating the appropriate use of key services and consistent performance in national quality measures.

Five stars for quality.

The Manor has received the highest overall quality rating – five stars – from the Centers for Medicare & Medicaid Services (CMS) Five-Star Quality Rating System. Only the top 10 percent of nursing homes in the country are eligible to earn this honor. The system rates the nation’s skilled nursing facilities scale based on the results of onsite surveys to determine compliance with federal and state regulations during a 12-month period. Facilities are rated on their performance in complying with health care standards, quality measures, and staffing.

A standard of excellence.

For the second consecutive year, The Manor has received the New Jersey Department of Health Advisory Standard of Care – a recognition of excellence and one of the top honors the state can bestow on a senior care facility. Licensing standards for New Jersey nursing homes include mandatory standards, which facilities must meet, and advisory standards, which are optional and voluntary. In addition to compliance with mandatory standards, The Manor achieved compliance with advisory standards in the areas of:

  • Access to care
  • Resident activities
  • Administration
  • Resident assessment and care plan
  • Communication
  • Dental services
  • Dietary services
  • Nurse staffing
  • Pharmacy
  • Quality assessment and medical records

The Manor received a deficiency-free score on the survey, which is a rare achievement for a senior care facility. The national average of deficiencies found on the survey is eight, while the average for New Jersey is six.
